요약: Helicobacter pylori (H. pylori) 감염이 확인된 환자를 대상으로 clarithromycin 500 mg, amoxicillin 1 g, 표준 용량의 proton pump inhibitor를 하루 2회 7~1일간 치료하는 표준 3제요법(standard triple therapy)은 현재 임상 영역에서 가장 흔하게 사용되는 치료법이다. 2013년 대한상부위장관헬리코박터학회에서 개정한 한국인 대상 H. pylori 감염 치료 가이드라인에서도 1차 제균 치료로 권고하고 있지만[1], 최근 H. pylori의 1차 제균 치료 성공률은 전 세계적으로 감소하고 있으며, 그 원인으로는 치료 항생제 중 특히 clarithromycin에 대한 내성 증가를 꼽을 수 있다. Maastricht V consensus에서는 clarithromycin 내성률이 15% 초과인 지역에서는 항생제 감수성 검사 없이, clarithromycin을 기본으로 한 3제요법을 1차 치료로 사용하지 않을 것을 주장하고 기존에 2차 요법으로 사용하던 비스무스(bismuth)를 기본으로 한 4제요법을 1차 치료에도 사용할 수 있도록 했지만[2], 실제 임상에 적용하는 데에는 문제가 적지 않다. 치료 도중에 발생하는 이상 사례의 발생 빈도가 상대적으로 높을 뿐 아니라, 새로운 항생제의 조합은 다른 예상하지 못하는 항생제의 내성을 키우기 때문에 그 지역의 항생제 내성의 빈도나 특징에 따라 다른 접근법을 가져야 한다.

Bismuth의 H. pylori 제균 효과는 이미 잘 알려져 있어서, H. pylori가 위 상피세포에 부착하는 것을 억제하고 직접적으로 H. pylori의 세포벽을 파괴하여 용해시킴으로써 제균 성공률을 높일 수 있다[3]. 2000년도 이후 bismuth를 포함한 1차제균 치료에 관한 임상 연구는 Table 1에 정리하였다[4-14]. 최근 몇몇 선행 연구에는 bismuth가 추가된 3제요법이 항생제 내성률이 상대적으로 높은 지역에서 효과적인 치료 성적을 보여주었고, 특히 주목할 점은 배양 검사를 통한 clarithromycin 내성균에 관한 연구들에서도 bismuth를 포함한 1차 제균요법은 86.7%의 성공률을 보인 반면 bismuth를 포함하지 않은 군에서는 33.3% 정도 성공률을 보였다는 것이다(OR, 10.64; 95% CI, 2.96~39.5; P<0.01) [15].

Clinical Trials of Bismuth-containing First-line Regimens for Helicobacter pyloris Since 2000

이 연구는 전향적으로 설계하였으며 H. pylori 1차제균 치료에 bismuth가 추가된 표준 3제요법의 효과를 확인하기로 하였을 뿐 아니라, 23S ribosomal RiboNucleic Acid (rRNA) point mutation로 확인된 clarithromycin 내성균주에서의 치료 효과를 알아보고자 하였다. 상기 연구는 전향적 단일기관 연구로서 2019년 1월부터 12월까지 총 107명의 H. pylori의 1차 제균 치료 환자를 대상으로 하였다. Bismuth (Tripotassiumdicitrate Bismuthate, DENOL Greencross Co., Seoul, Korea; 300 mg b.i.d)가 추가된 표준 3제요법으로 14일간 1차 제균 치료를 시행하였다. 동시에 환자들은 중합효소 연쇄반응(PCR)을 이용하여 clarithromycin 내성 여부를 확인하였고, 이에 대한 bismuth 추가 표준 3제요법의 치료 효과를 clarithromycin 내성 여부에 따라 세부적으로 평가하였다. 이중 104명의 환자가 제균 치료 성공 여부를 확인할 수 있었으며, H. pylori 제균 성공률은 각각 intention-to-treat (ITT) 분석에서 87.9%, per-protocol (PP) 분석에서 90.4%였다. Clarithromycin 내성 여부를 확인하기 위한 PCR에서 23S rRNA point mutation의 빈도는 33.6% (35/104)였으며, A2143G는 가장 높은 빈도의 point mutation으로 나타났다(85.7%, 30/35). Clarithromycin 내성 여부에 따라 세부 분석해 보았을 때, 내성이 없는 환자에서는 97.1% (67/69)의 높은 제균 성공률을 보였으며, 내성이 있는 환자에서는 77.1% (27/35)의 제균 성공률을 보여주었다. 또한 고무적인 것은 1차 제균에 실패한 10명의 환자가 bismuth 포함 기본 4제요법으로 2차 치료에서 모두 제균에 성공하여 3차 구제요법까지 진행할 필요가 없었다는 것이다. 다만, 국내 기존의 연구 결과에서 표준 3제요법의 제균 성공률이 70% 내외인 점을 고려하여 단일군 연구(single-arm trials)로 실시하여 실제 비교 자료를 제시할 수 없었고, 이러한 면에서 제한점이 있었다. 하지만, 이번 연구를 통하여 고식적으로 널리 쓰이는 1차 제균 치료의 부족한 임상 효과를 극복하기위해 표준 3제요법 치료에 bismuth 제재를 추가하는 것이 효과적이었고, clarithrymycin 내성을 극복하는 측면에서도 임상적으로 받아들일 수 있는 정도였다는 메시지를 주었다. 앞으로 대규모의 전향적 비교 임상 연구를 기대해 본다.

해설: 최근 H. pylori에 대한 표준 3제요법의 제균 성공률은 전세계적으로 70%까지도 보고하고 있고, 국내에서도 제균 성공이 ITT 분석에서 70.7% (58.7∼80.0%), PP 분석에서 76.2% (64.5∼87.5%)로 감소하였다[16,17]. 이러한 기대 이하의 결과는 clarithromycin 내성균의 증가에서 기인하는 것으로 보고 있다. 우리나라에서 clarithromycin 내성률은 2000년 이전에는 5.9% 정도였으나, 2003년에는 13.8%, 이후 2007~2009년에는 37.0%로 꾸준히 증가하는 추세에 있다. 최근 10년 이내에 수행된 연구의 clarithromycin에 대한 내성률도 38.5%로 증가하였다는 점은 놀라운 일이나, 지역별 편차가 크다는 점도 주목을 해야 할 것이다[18,19].

표준 1차 치료법의 제균 성공률 감소를 극복하기 위해서 가장 먼저 선택할 수 있는 방법으로는 제균 치료 기간을 연장하는 것을 고려해볼 수 있는데, 단순히 제균 치료 기간만을 연장하는 것으로 제균 성공률을 높일 수 있을지는 논란의 여지가 있다. 기존의 메타분석에 의하면 7일에 비해서 10일로 연장하면 제균 성공률이 4%, 14일로 연장하면 5~6% 정도의 기대 이하의 향상만이 있어 아시아-태평양 권고안에서는 14일로 연장하는 것이 일부 제한적인 효과를 보여주는 것으로 명시되어 있다[20]. 반면 2020년 개정된 한국인 Helicobacter 감염 치료 근거 기반 임상진료지침 개정안에서는 14일 치료의 제균 성공률이 7일이나 10일 치료에 비해 유의하게 높았다. 하지만, 표준3제요법의 ITT 제균 성공률이 14일 치료에서 78.1% (95% CI, 75.2~80.7%) 정도임을 감안하면 현재 시점의 문제를 해결하는 데에는 한계가 있을 것으로 보인다[21]. 다음으로는 항균보조제 혹은 유산균을 추가할 수 있는데, 그 기전은 H. pylori에 대한 직접 억제 효과, 점막 방어 강화 효과, 면역 반응 조절 효과, Ig A 분비 등과 함께 항생제 사용과 연관된 부작용을 감소시켜 약제 복용 순응도를 높이는 것 등으로 제안하고 있다[22]. 여러 가지 이론적 배경으로 확실한 효과를 기대하였지만, 현재까지의 연구들을 종합해보면 그 효과가 기대에 미치지 못하고 있으며 그 역할 또한 제한될 수밖에 없다. 다른 한편으로는 항생제 조합의 차이를 주는 방법이 있는데, 이러한 대안적 1차 제균요법들조차 기존의 clarithromycin과 amoxicillin을 기반으로 한 표준 1차 제균요법의 한계를 획기적으로 극복하였다고 보기는 어렵다. 따라서 임상진료지침 개정안에서도 표준 1차 제균 요법을 초치료로 그대로 유지하면서 동시 치료 및 bismuth 기반 4제요법 등을 1차 제균 치료의 한 가지 방법으로 인정하는 수준에서 채택하고 있는 실정이다[21]. 더욱이 2016년 교토 consensus와 2018년 건강보험 확대에 따라 급격히 늘어난 H. pylori 제균 치료로 인하여 2차 제균 치료요법으로 bismuth 기반으로 하는 4제요법의 빈도가 늘어나고 있으며, 10년 전에 90% 이상의 높은 제균 성공률에 비해 점진적으로 감소하는 경향을 보이고 있는 점은 우리에게 시사하는 바가 크다[23]. 이로 짐작해 볼 수 있는 바는 기존의 bismuth 기반으로 하는 4제요법을 1차 치료로 전환시켰을 때 나타날 수 있는 새로운 항생제 내성의 가능성을 우려하는 것과 같다.

국내에서는 아직 clarithromycin 내성이 확인된 환자에서 1차 치료 약제에 대한 표준 가이드라인은 정립되지 않았다. 순차요법(sequential therapy), metronidazole을 포함하는 표준 3제요법(metronidazole triple therapy) 그리고 동시요법(concomitant therapy) 등이 연구 조사되었으나 모두 만족할 만한 성적을 이루지 못했다[24]. 순차요법의 경우에서는 다른 요법에 비해 효과가 현저히 떨어졌으며, metronidazole 포함 3제요법이나 동시요법은 clarithromycin과 metronidazole resistance가 같이 공존하고 있는 내성균에서는 효과가 제한적이라는 것이다[25]. 이러한 점을 극복하기 위해 14일 요법을 일반적으로 사용하는 경우에는 그 부작용을 감당하기 부담스러울 수 있다. 최근 vonoprazan을 기본으로 하는 3제요법이 PPI에 비해 clarithromycin 내성균에 대해 탁월한 효과를 보였다는 점은 주목할 만하다(pooled eradication rates, 82.0% vs. 40.0%; OR, 6.83; 95% CI, 3.63~12.86; P<0.01) [26]. 그러나 검증의 시간을 필요로 하고, 현재 우리나라에서는 사용할 수 없다는 것이 문제일 것이다.

Bismuth는 화학 원소로 기호는 Bi, 원자 번호는 83이며, 중금속으로 분류되지만 독성은 거의 없다. 예전부터 그 화합물은 화장품이나 의약물로 사용하였고, H. pylori 제균 치료에도 사용하고 있지만, 현재까지 그 내성이 보고된 바가 없다. 제균 치료에 사용되는 기전으로 현재 제시하고 있는 것은 bismuth 제재가 bacterial wall이나 periplasmic space에서 complex를 형성하여 효소의 작용이나 adenosine triphosphate 생성을 억제하고, 위벽에 세균이 부착하는 것을 방해하면서 직접적인 살균 효과를 나타낸다는 것이다. 또한, prostaglandin, epidermal growth factor 등 점막 보호 인자를 증강시켜 궤양의 호전을 촉진시킬 뿐만 아니라 metronidazole, clarithromycin 등 항생제와 synergic effect를 일으키는 것으로 알려져 내성이 확인된 균주에서도 제균 치료 성적을 향상시킬 수 있을 것으로 기대하고 있다[27,28]. 그러나 단독으로 치료하게 되면 그 제균 효과는 적어서 다른 항생제와의 병합요법으로 이용되었고 1차 치료보다는 주로 2차 제균 치료에 사용되는 약제로 사용되었다. 최근 bismuth를 제균요법에 추가할 때의 임상 결과를 종합한 메타분석 결과를 보면 bismuth를 1차 제균 치료에 추가하였을 때, 기존의 치료에 비해 3.5배의 치료 효과 상승을 보였다. 또한 우려하였던 약제 부작용이나 복약 순응도에서도 bismuth를 추가한 경우 그렇지 않은 경우에 비해 크게 다르지 않았다는 점이다[15]. 이러한 점들을 고려하였을 때, clarithromycin 내성의 빈도가 높다고 알려진 지역에서 이에 대한 전향적 연구의 필요성이 고조되었고, 기존의 연구에서는 이에 대한 고려가 부족한 실정이었다[29].

이에 본 연구는 bismuth를 기존의 표준 1차 제균 치료요법에 추가하는 것만으로도 제균 성공률을 90%까지 높일 수 있음을 보여주고, clarithromycin 내성이 있는 환자에서조차도 77% 정도의 효과를 나타내어 임상적으로 받아들일 정도로 나왔다. 또한 우려했던 부작용의 빈도도 그리 높지 않아 현실적으로 clarithromycin 내성 검사에 기반한 맞춤형 치료가 어려운 실제 임상 현장에서 하나의 대안적 방안이 될 수 있을 것이라고 기대한다.
