Abstract

Background/Aims

Helicobacter pylori (H. pylori) can disseminate between couples. The present study compared the findings of gastric cancer screening between seronegative subjects according to the presence of an infected spouse.

Materials and Methods

Follow-up data of seronegative subjects were analyzed among married couples who underwent gastric cancer screening via gastroscopy, serum pepsinogen, and anti-H. pylori IgG assays between January 2010 and May 2016. New detection rates of H. pylori infection and gastric neoplasm at the follow-up screening were compared between seronegative subjects according to the H. pylori-infected status of spouse.

Results

Among 246 seronegative subjects with an H. pylori-infected spouse, 92 underwent follow-up tests (case group). Among 278 seronegative subjects with seronegative spouse, 94 underwent follow-up tests (control group). The past infection rate was higher in the case group than in the control group (52/92 vs. 34/94; P=0.005). New H. pylori infection was diagnosed in three of the 92 cases and two of the 94 controls (3.2% vs. 2.1%; P=0.681). During the mean follow-up of 67.9±36.0 months, three adenocarcinomas and two adenomas (5/184) were newly detected among the cases and their spouses, whereas none (0/188) were detected among the controls and their spouses (2.7% vs. 0%; P=0.029).

Conclusions

Gastric neoplasm occurred more frequently in couples with an H. pylori-infected spouse. Because the past infection rate is higher among seronegative subjects with an infected spouse, gastric cancer screening is recommended in both partners when the spouse is infected.

서 론

위암의 가족력이 있는 한국인에서 헬리코박터 파일로리(Helicobacter pylori, H. pylori) 제균 치료는 위암 발생률을 낮추는 데 도움이 된다[1]. H. pylori균의 가족 내 전염은 주로 부모로부터 자녀에게 전파되거나 형제 간의 전염으로 발생한다[2]. 부부 간의 전염은 이보다 드물지만 H. pylori 감염률이 높은 집단에서는 부부 간에 전염될 위험성도 높다[3]. 최근 메타분석 연구에 의하면 제균 치료 후에 가족 내 감염자가 재감염의 원인이 될 수 있으므로, 다른 가족 내 감염자들도 함께 치료하는 것이 좋다[4].

부부가 모두 H. pylori 감염자인 경우, 둘 다 위암이 발생하는 경우도 있지만[5], 위암화 과정에는 유전과 환경 및 식이 등 복합적인 요인이 관여하므로 감염자의 예후는 다양하다[6]. 위암 환자를 H. pylori 감염 상태별로 분류한 국내 연구에 의하면 75.2%가 현감염자, 22.5%가 과거감염자, 2.3%가 미감염자였다[7]. 과거감염자는 현감염자에 비해 위암 발생률이 낮지만, 미감염자에 비해서 높기 때문에 제균 후에는 정기적인 위내시경 추적 검사가 필요하다[8].

한국인 부부 중에는 한 명만 감염된 경우가 흔하지만, 감염된 배우자가 감염이 없는 배우자에게 미치는 영향에 대한 연구는 드물다. 또한, 국내에서 시행되는 위내시경 검사 결과의 85.9%가 위염으로 진단되고 있고, 대부분이 표재성 위염(31.3%), 위축성 위염(27.1%), 미란성 위염(23.7%)으로 진단되어[9], 진단명만으로 감염 상태나 위암의 위험도를 알기 어려운 경우가 많다. 즉, 위암의 고위험군 선별에 도움이 되지 않는다고 하여 위염의 쿄토 분류에서 삭제된 표재성 위염이나 미란성 위염이 아직도 위내시경 진단명으로 사용되고 있고, 위축성 위염 진단 시에는 폐쇄형이나 개방형의 단계가 기재되어 있지 않아서 고위험군을 선별하는 데 한계가 있다[10]. 이에 연구자들은 부부가 동시에 본원에서 위내시경 검사와 혈청 항H. pylori IgG 및 펩시노겐(pepsinogen, PG) 검사를 받은 후, 추적 위암 검진을 받은 한국인 부부들의 예후를 조사하였다. 구체적인 연구 목표는 감염이 없는 혈청 항H. pylori IgG 음성자(이하 혈청항체 음성자)가 감염자인 배우자와 살 때, 추적 검사에서 새로운 H. pylori 감염이나 위암이 발생할 가능성이 높은지를 조사하는 것이었다.

대상 및 방법

1. 연구 대상군과 대조군의 선정

이 환자-대조군 관찰 연구는 부부가 동시에 본원에서 2010년 1월부터 2016년 5월 사이에 위암 검진을 목적으로 위내시경과 혈청 PG 검사 및 항H. pylori IgG 검사를 받은 뒤, 추적 검진을 받은 한국인 부부를 대상으로 했다. 대상군(case group)은 감염이 없는 것으로 판정된 혈청항체 음성자 중에서 같은 날 검진을 받은 배우자가 H. pylori 감염으로 진단된 경우로 했다. 대조군(control group)은 감염이 없는 혈청항체 음성자 중에서 같은 날 배우자도 혈청항체 음성으로 보고된 경우로 했다. 부부 중 한 명이라도 위내시경이나 혈청 검사를 시행하지 않거나, 추적 검사를 받지 않은 경우는 분석에서 제외하였다. 또한, 치료를 요하는 질환 및 3개월 이내의 약물 복용력을 조사하여, IgG 형성에 장애가 있는 희귀질환이 있거나 항생제를 복용한 경우는 연구에서 제외했다. 이 연구는 본원에서 혈청학적 위축이 없는 혈청항체 음성자 2,620명에 대한 후속 연구로[11], 이후에 시행된 검사 결과에 대한 수집과 분석에 대해서는 본원 윤리위원회의 승인을 받아 진행했다(KUH 1010993).

2. 혈청 검사

혈청 항H. pylori IgG와 PG 검사를 위한 채혈은 12시간 금식한 상태에서 위내시경 검사 전에 시행되었다. 혈청 PG 검사는 기존 연구와 같이 latex enhanced turbidimetic immunoassay (HBi Co., Ltd., Anyang, Korea) 검사로 PG I 수치, PG II 수치, PG I/II 비를 측정했으며, 혈청학적 위축(serologic atrophy 또는 gastric corpus atrophy)은 PG I/II 비가 3.0 이하이면서 PG I 수치가 70 ng/mL 이하일 때로 했다[12].

혈청항체 검사는 한국인에서 민감도와 특이도가 각각 100%와 75.4%, 89.7%와 85.5%로 보고된 Chorus H. pylori IgG(DIESSE Diagnostica Senese, Siena, Italy)와 Vidas H. pylori IgG (BioMérieux, Marcy-l’Etoile, France)로 시행했다[13]. 혈청항체양성은 Vidas H. pylori IgG 검사에서 test value가 1.0 이상이거나 Chorus H. pylori IgG 검사에서 arbitrary unit (AU)가 12 AU/mL 이상일 때 진단했다.

3. 상부위장관 내시경 검사

위내시경 검사는 GIF-H260나 GIF-H290 (Olympus, Tokyo, Japan) 또는 EG-2990i (Pentax, Tokyo, Japan)로 진행되었다. 위내시경 소견은 위염의 교토 분류에 맞춰서 다음과 같이 진단했다[14]. 정상 혈관상(regular arrangement of collecting venules, RAC)은 새 발자국 모양의 짧은 붉은색 혈관상이 위각과 체부의 소만측에서 균일하게 관찰될 때 진단하였다. 기저부에서 발적이 관찰되면 광범위한 발적(diffuse redness)으로 진단하였고, 작은 다발성 혈성 반점까지 보이면 점상 발적(spotty redness)을 추가했다. 전정부에서 닭살 모양의 결절들이 관찰되면 결절성 위염(nodular gastritis)으로 진단하였고, 주름이 5 mm 이상으로 두꺼워져 있으면 주름 비대(hypertrophic rugae)로 진단했다. 황색종(xanthoma)은 노란색 반점이 국소적으로 보일 때 진단했으며, 화생성 위염(metaplastic gastritis)은 울퉁불퉁한 흰색 융기나 붉은 함몰 또는 융모상 변화(villous change)가 불규칙하게 관찰될 때 진단했다. 위축성 위염(chronic atrophic gastritis, CAG)은 점막이 얇아져서 점막하층이 salt-and-pepper 모양으로 투영되거나[15], 점막하층의 혈관상이 보라색의 긴 나뭇가지 모양으로 관찰되면 진단했다. CAG의 분류는 최근에 보고한 연구처럼 폐쇄형(closed-type 0, 1, 2, 3)부터 개방형(open-type 1, 2, 3, pangastritis)까지 8단계로 분류했다[16]. 위 내용을 교토 분류점수체계에 맞춰서 0점부터 8점까지 채점했다. 위축성 위염은 A0 (폐쇄형 0-1), A1 (폐쇄형 2-3), A2 (개방형)로 채점했으며, 화생성 위염은 IM0 (없음), IM1 (전정부에만 있음), IM2 (체부에도 있음)로 했다. 기저부의 광범위한 발적은 DR0 (없음), DR1 (약함), DR2 (심함)로 채점했다. 주름 비대는 H0 (없음)과 H1 (있음)으로 했으며, 결절성 위염은 N0 (없음)과 N1 (있음)으로 채점했다.

4. H. pylori 감염 상태의 진단

현감염은 위내시경 조직 검사로 시행한 신속요소분해효소 검사(rapid urease test)나 Giemsa 염색 또는 요소호기 검사(13C-urea breath test)에서 양성 소견을 보이면 진단했다. 감염자 5명 중 1명은 조직 검사에서 H. pylori 위 음성으로 나올 수 있으므로[17], 혈청항체 양성자의 위내시경 소견에서 전형적인 점상 발적이 관찰되면 감염자로 분류했다[18,19].

과거감염은 혈청항체 음성자가 제균 치료를 받은 적이 있거나 위내시경에서 황색종이나 화생성 위염 또는 전정부에서 체부까지 진행된 폐쇄형 2형 이상의 위축성 위염이 있으면 진단하였다. 조직 검사에서 장상피화생이 관찰되거나 혈청학적 위축이 진단된 경우는 균의 자가 소멸(spontaneous eradication)이나 의도하지 않은 제균(unintended eradication)으로 판단하여 과거감염자로 분류했다[20].

미감염은 위에 기술된 현감염이나 과거감염의 기준에 해당하지 않는 혈청항체 음성자 중에서 H. pylori 감염으로 진단받은적이 없고, 시행된 H. pylori 검사에서 모두 음성 소견을 보이며, 위내시경에서 RAC가 관찰되면 진단하였다[21]. 본인도 모르는 사이에 앓고 지나간 과거감염자를 배제하기 위해서 위와 같이 엄격한 기준을 사용했다.

5. 추적 검사 결과 및 연구 결과지표

위내시경과 혈청 검사는 첫 검진 시와 동일한 방법으로 진행되었고, 같은 기준으로 판독되었다. Vidas H. pylori IgG 검사는 2012년까지만 사용되었고, 이후에는 Chorus H. pylori IgG 검사가 사용되었다. 연구의 일차결과지표(primary study outcome)는 대상군과 대조군에서 새로 발생한 H. pylori 감염으로 하였다. 이차결과지표(secondary study outcome)는 대상군과 대조군 및 각 배우자군에서 새로 발생한 선암이나 선종으로 하였다. 결과지표물이 나온 경우는 진단된 시점까지를 추적 검사기간으로 했으며, 결과지표물이 없는 경우는 가장 최근의 위내시경 검사일까지를 추적 검사 기간으로 했다.

6. 통계 분석

연속형 변수는 student’s t-test로 분석해서 평균±표준편차(standard deviation)로 표시하였으며, 범주형 변수는 chi-square test로 분석해서 빈도(%)로 표시하였다. 정규 분포를 하지 않는 연속형 변수는 Kruskal-wallis test로 분석해서 중앙값(최소치-최고치)으로 표시했다. 정규 분포를 하지 않는 범주형 변수는 Fisher’s exact test로 분석해서 빈도(%)로 표시했다. 통계 분석은 PASW Statistics version 24.0 (SPSS Inc., Chicago, IL, USA)으로 했으며, 유의수준은 P값이 <0.05일 때로 하였다.

나아가서 부부를 연령대별로 분류한 뒤, 30대, 40대, 50대, 60대, 70대 부부 간의 차이는 chi-square test와 Bonferroni correction으로 분석하였으며, multiple testing correction 후 유의수준은 P값이 <0.01일 때로 하였다. 위염의 교토 분류에 대한 관찰자 간의 차이는 Kappa (κ)값을 분석한 뒤, standard error (SE)와 함께 기술했으며, 유의수준은 P값이 <0.05일 때로 했다. 매우 높은 일치도(excellent agreement)는 κ>0.80, 높은 일치도(good agreement)는 κ>0.60~0.80, 중등도의 일치도(moderate agreement)는 κ>0.40~0.60, 낮은 일치도(fair agreement)는 κ>0.20~0.40, 매우 낮은 일치도(poor agreement)는 κ≤0.20로 했다.

결 과

1. 대상군과 대조군의 비교

연구 기간 동안 본원에서 691쌍의 부부가 위암 검진으로 혈청 검사와 위내시경 검사를 받았다. 그 결과, 246명의 혈청항체 음성자는 배우자가 H. pylori 감염자로 진단되었고, 278명의 혈청항체 음성자는 배우자도 혈청항체 음성으로 보고되었다. 이 중에서 대상군 92명과 대조군 94명이 배우자와 함께 본원에서 추적 검진을 받아서 연구에 포함되었다(Fig. 1).

대상군과 대조군의 성별과 나이 및 혈청 검사 결과에는 유의한 차이가 없었다(Table 1). 대상군 중 과거감염자 비율은 56.5% (52/92)로 대조군의 36.2% (34/94)에 비해서 높았다(P=0.005). 과거감염자로 분류된 대상군 52명 중 9명은 제균 치료를 받은 적이 있었고, 19명은 화생성 위염, 15명은 폐쇄형 2형 이상의 위축성 위염, 3명은 황색종이 진단되었다. 이외에 4명이 조직 검사에서 장상피화생으로 보고되었고, 2명이 혈청 검사에서 혈청학적 위축으로 진단되어 과거감염자로 추가되었다. 한편, 과거감염자로 분류된 대조군 34명 중 9명은 제균 치료를 받은 적이 있었고, 13명은 화생성 위염, 8명은 폐쇄형 2형 이상의 위축성 위염, 1명은 황색종이 진단되었다. 이외에 2명은 장상피화생이 조직 검사로 진단되었고, 1명은 혈청학적 위축이 진단되어 과거감염자로 추가되었다.

Baseline Characteristics of Seronegative Subjects who Underwent Gastric Cancer Screening Tests with Their Spouse

대상군의 위축성 위염 점수가 대조군에 비해서 높아서(0.46 vs. 0.24, P=0.004), 교토 분류 점수의 합도 대상군이 대조군에 비해 높았다(0.84 vs. 0.46, P=0.010). 두 관찰자(J.M.P., S.Y.L.) 간의 위염 점수 차이를 분석한 결과, 위축성 위염 점수는 대상군 3명과 대조군 5명에서 차이를 보였으며(3.3% vs. 5.3%, P=0.721), κ값은 0.910로 매우 높은 일치도를 보였다(SE=0.030, P<0.001). 화생성 위염 점수는 대조군 3명에서만 차이를 보였으며(0% vs. 3.2%, P=0.246), κ값은 0.967로 매우 높은 일치도를 보였다(SE=0.019, P<0.001). 광범위한 발적, 주름 비대, 결절성 위염에 대한 점수는 해당 예가 없어서 0점으로 일치하였다. 교토 분류 점수의 합에 대한 κ값은 0.916로 관찰자 간에 매우 높은 일치도를 보였다(SE=0.025, P<0.001).

2. 대상군의 배우자와 대조군의 배우자 간의 비교

대상군의 배우자 92명과 대조군의 배우자 94명을 비교한 결과, 성별과 나이에 유의한 차이는 없었다(Table 2). 대상군의 배우자 92명은 모두 현감염자로, 대조군의 배우자 94명에 비해 유의하게 높은 혈청항체가를 보였다(Vidas: 3.45±0.77 vs. 0.46±0.23 AU/mL, P<0.001; Chorus: 150.7±48.9 vs. 6.6±1.8 AU/mL, P<0.001). 대조군의 배우자 94명 중 33명(35.1%)은 과거감염자이자, 61명은 미감염자로 분류되었다. 과거감염자 33명 중 11명은 제균 치료를 받은 적이 있었고, 12명은 화생성 위염, 4명은 폐쇄형 2형 이상의 위축성 위염, 2명은 황색종이 진단되었다. 2명은 조직 검사에서 장상피화생이 진단되어 과거감염자로 추가되었다.

Baseline Characteristics of the Spouses of Case and Control Groups

대상군의 배우자의 평균 혈청 PG I 수치(68.5±33.3 vs. 48.5±19.9 ng/mL, P<0.001)와 PG II 수치(20.9±9.4 vs. 8.7±3.4 ng/mL, P<0.001)는 대조군의 배우자에 비해서 유의하게 높았다. 아울러 혈청 PG I/II 비는 3.4±1.3로, 대조군의 배우자의 5.8±1.4에 비해서 유의하게 낮았다(P<0.001).

대상군의 배우자는 광범위한 발적 점수(1.66 vs. 0, P<0.001), 위축성 위염 점수(1.41 vs. 0.33, P<0.001), 화생성 위염 점수(1.11 vs. 0.32, P<0.001)가 대조군의 배우자에 비해 높아서 위염의 교토 분류 점수에서 높은 점수를 보였다(4.22 vs. 0.46, P<0.001). 두 관찰자(J.M.P., S.Y.L.) 간의 위염 점수 차이를 분석한 결과, 위축성 위염 점수는 대상군의 배우자 5명과 대조군의 배우자 1명에서 차이를 보였으며(5.4% vs. 1.1%, P=0.116), κ값은 0.942로 매우 높은 일치도를 보였다(SE=0.022, P<0.001). 화생성 위염 점수는 대상군의 배우자 3명에서만 차이를 보였으며(3.3% vs. 0%, P=0.119), κ값은 0.966로 매우 높은 일치도를 보였다(SE=0.017, P<0.001). 광범위한 발적 점수는 대상군의 배우자 2명에서만 차이를 보였으며(2.2% vs. 0%, P=0.243), κ값은 0.982로 매우 높은 일치도를 보였다(SE=0.013, P<0.001). 주름 비대 점수와 결절성 위염 점수는 해당 예가 없어서 0점으로 모두 일치하였다. 교토 분류 점수의 합에 대한 κ값은 0.939로 매우 높은 일치도를 보였다(SE=0.020, P<0.001).

3. 추적 검사 기간 중 감염 상태가 변한 수진자

평균 추적 검사 기간은 67.9±36.0개월로 양 군 간의 차이는 없었다(66.0±35.8개월 vs. 69.7±36.2개월, P=0.486). 위내시경 검사를 받은 평균 횟수는 대상군은 3.13±0.99회, 대조군 부부에서 3.12±0.85회로 차이가 없었다(P=0.921).

추적 검사에서 새로 감염이 진단된 사람은 5명이었다(Table 3). 대상군 92명 중 3명이 추적 검사에서 새로운 감염이 진단되었으며, 대조군 94명 중 2명이 진단되어 차이가 없었다(3.2% vs. 2.1%, P=0.681). 새로 감염된 대상군 3명은 모두 과거감염자로, 현감염자인 배우자를 두고 있었다. 이들 중 과거감염자인 남편 2명에게는 감염된 부인이 있었고, 과거감염자인 부인 1명에게는 위암 절제술을 받은 감염자 남편이 있었다. 한편, 새로 감염된 대조군 2명 중 1명은 과거감염자인 남편으로 감염력이 없는 부인이 있었다. 나머지 1명은 부부 모두 감염력이 없는 상태에서 남편만 새로 감염이 진단되었다.

Seronegative Subjects with New H. pylori Infection Detected at the Follow-up Screening Tests

현감염자인 대상군의 배우자 92명 중 9명이 추적 기간 중에 제균되었다. 감염이 지속된 83명과 제균된 9명의 배우자인 대상군에서의 새로운 감염률은 3.6% (3/83) 대 0% (0/9)로 차이가 없었으며(P>0.999), 평균 나이(55.4±10.5세 vs. 50.7±7.8세, P=0.116)나 추적 기간(64.4±34.0 vs. 84.0±44.9, P=0.192)에도 차이가 없었다.

4. 추적 검사에서 위암이나 선종이 진단된 수진자

추적 위내시경 조직 검사에서 선암이 진단된 사람은 3명이었으며, 선종이 진단된 사람은 2명이었다(Table 4). 5명 모두 대상군 92명과 그 배우자 92명에 속했다. 한편, 대조군 94명과 배우자 94명 중에서는 위종양이 진단되지 않아(0%), 대상군 92명과 배우자 92명의 위종양 발생률(2.7%)과 유의한 차이를 보였다(P=0.029). 감염자 83명과 제균된 9명에서의 위종양 발생률은 2.4% (2/83) 대 0% (0/9)로 차이가 없었다(P>0.999).

New Gastric Neoplasms Detected During the Follow-up Screening Tests

진단된 암은 모두 장형 위선암으로, 현감염자인 배우자 2명과 과거감염자인 대상군 1명에서 각각 발견되었다(Fig. 2). 현감염자 2명은 T2기의 중분화도 선암으로 진단되었으며, 과거감염자 1명은 T1a기의 저분화도 선암으로 진단되었다. 선종은 과거감염자인 대상군 1명과 현감염자인 배우자 1명에서 진단되었는데, 각각 혈청 PG I/II 비가 1.1과 1.0으로 혈청학적 위축이 심한 상태였다.

Final diagnosis was intestinal-type, AGC B3 consisting of moderately differentiated adenocarcinoma. He had infection, whereas his wife had no history of infection. 5. 연령대별로 본 감염 상태의 차이

모든 부부를 30대, 40대, 50대, 60대, 70대의 5개군으로 분류한 뒤, 종양 발생률과 재감염률 및 감염 상태를 조사한 결과, 항체 음성자로 구성된 세 군(대상군, 대조군, 대조군의 배우자군) 모두에서 30대의 과거감염률이 10%대로 가장 낮았으며, 70대는 80% 이상으로 가장 높았다(Table 5). 연령대별 과거감염자의 비율 차이를 비교한 결과, 50대 부부에서만 유의한 차이를 보였다(Fig. 3). 50대 대상군 부부의 과거감염률은 67.6%로, 대조군의 30.3% (P=0.001)와 대상군의 배우자군의 27.3% (P=0.001)에 비해서 유의하게 높았다.

고 찰

H. pylori 감염이 없는 혈청항체 음성자의 부부 검진 결과를 5.7년간 추적한 결과, 감염자가 있는 부부에서 위암이나 선종이 발생할 확률은 2.7%로, 감염자가 없는 부부의 0%에 비해 높았다. 감염자가 있는 부부는 혈청항체 음성자인 배우자의 56.5%가 과거감염자였지만, 감염자가 없는 부부는 36.2%만 과거감염자라서 발생률에 차이를 보인 것으로 추정된다. 한편, 새로운 감염 발생률은 양 군 간에 차이가 없었다.

이 연구를 통해서 확인된 중요한 점은 같은 혈청항체 음성자라도 감염자인 배우자를 둔 사람은 과거감염자일 가능성이 높아서 위종양 발생률이 높다는 것이다. 종양이 발생한 혈청항체 음성자 2명은 모두 과거감염자로, 제균 후에도 혈청학적 위축이 회복되지 않은 66세 남성에서는 선종이 발생했지만, 46세 여성은 혈청학적 위축이 없는 상태에서 저분화도의 선암이 발생하는 대조적인 소견을 보였다. 이는 위산 분비능이 저하된 상태에서는 선종이나 장형 위암이 발생하기 쉽고[22], 위산 분비능이 손상되지 않은 젊은 폐경 전 여성일수록 미만형 위암이 발생하기 쉽기 때문으로 해석된다[23]. 일반적으로 과거감염자에서 발생하는 위암은 현감염자에서 발생하는 위암보다 예후가 양호하다[24]. 따라서 이 연구에서 발견된 새로운 저분화도의 선암도 점막층에 국한된 장형 위암 상태에서 진단된 것으로 추정된다.

위암 발생률이 가장 높았던 집단은 대상군의 배우자군으로, 이는 현감염자에서 발생률이 가장 높기 때문으로 해석된다[8]. 감염자를 9년 이상 추적한 국내 연구에서도 제균 치료를 받은 608명 중에서는 5명(0.8%)에서만 위암이 발생했지만, 제균하지 않은 979명 중에서는 28명(2.9%)에서 위암이 발생했다[25]. 이 연구에서 감염자 2명에서 진행성 위암이 새로 진단되었고 1명에서 선종이 진단되었는데, 같은 감염자라도 고령이면서 PG I 수치가 6.1 ng/mL, PG I/II 비가 1.0으로 혈청학적 위축이 심했던 78세 남성에서는 다발성 위 선종이 발생한 반면, 65세 남성과 67세 남성에서는 진행성 위암이 발생했다. 진행성 위암이 진단된 67세 남성도 혈청학적 위축이 있었으나, PG I 수치가 44.5 ng/mL, PG I/II 비가 2.7로 선종이 발생한 78세 남성에 비해 위축이 덜 심한 편이어서, 선종이 아닌 장형 위암이 발생한 것으로 추정된다.

부부가 모두 혈청항체 음성자인 경우, 위암이나 선종이 발생하지 않았다는 점도 흥미롭다. 이 연구에서 혈청항체 음성자로만 구성된 세 집단의 PG 결과를 비교해보면 PG I/II 비가 대조군의 배우자군(5.8±1.4)>대조군(5.7±1.5)>대상군(5.4±1.5) 순으로 높았다. 혈청 PG I/II 비가 낮을수록 장형 위암이 발생하기 쉬우므로[22], 앞으로도 위암 발생률은 대조군의 배우자군에서 가장 낮을 것이다. 한편, PG II 수치가 높을수록 미만형 위암이 발생하기 쉬운데[23], 대조군 배우자군의 PG II 수치(8.7±3.4 ng/mL)가 대조군(9.3±5.8 ng/mL)이나 대상군(9.3±3.7 ng/mL)보다 낮아서 대조군의 배우자군이 가장 좋은 예후를 보일 것으로 예상된다.

위종양이 발생한 과거감염자 2명은 배우자가 혈청학적 위축 소견을 보이는 감염자로, 모두 혈청항체 수치가 최고치를 보이는 활동성 감염 상태였다. 이들 배우자의 혈청 PG II 수치는 각각 24.1 ng/mL과 22.1 ng/mL로 높아서 미만형 위암의 위험성이 높은 상태이면서[23], 혈청학적 위축까지 있어 장형 위암의 위험성도 높은 상태였다[22]. 이들이 과거감염자인 배우자의 위종양 발생에 어떤 영향을 주었는지를 직접적으로 증명하기는 어렵지만, 부부가 동시에 감염된 상태에서 오래 지내다가 한 사람만 과거감염자가 되고, 과거감염 상태에서 예후가 양호한 선종과 장형 위암이 발생한 것으로 추정된다.

배우자가 감염력이 없어도 위종양이 발생한 경우는 65세 남성과 78세 남성의 두 증례가 있었다. 환자들이 배우자가 아닌 다른 사람에게 전염되었을 가능성도 있지만[26], 배우자들이 본인도 모르는 사이에 감염되었다가 제균된 후에 위점막이 회복되어 검사 결과가 마치 미감염자처럼 보였을 가능성도 있다[27]. 아울러 새로운 감염자들 중 2명은 배우자가 미감염 상태였지만, 43세 남성은 과거감염자였기에 39세인 배우자는 본인도 모르는 사이에 감염되었다가 회복된 과거감염자일 가능성이 있다. 나머지 3명은 감염자인 배우자를 둔 과거감염자에서 발생한 재감염이었기에 배우자로부터 전염되었을 가능성을 배제할 수는 없다. 나아가서 감염자 83명과 제균된 9명의 위종양 발생률이나 배우자의 감염률에 차이가 없었는데, 위암화 과정은 오랜 시간을 필요로 하기에 감염자가 제균되어도 상대 배우자에게 미치는 영향력이 5.7년 안에 바로 나타나지 않기 때문으로 생각된다.

이번 연구에서 50~59세의 과거감염률이 대상군에서 67.6%로 대조군이나 대조군의 배우자에 비해서 유독 높았는데, 50대 부부의 동거 기간이 20년 전후라는 점을 감안할 때, 대상군에서의 높은 과거감염률이 감염자인 배우자에 의한 수평전염 때문일 가능성이 있다. 만약 대부분의 과거감염이 유아기에 걸린 수직전염 때문이라면 대상군의 50대 부부의 과거감염률도 대조군이나 대조군의 배우자들의 50대 부부처럼 30% 전후여야 하지만, 감염자인 배우자를 둔 대상군만 2배 이상 높다는 것은 수평전염이 수직전염만큼 있었다는 것을 시사한다. 서양에서는 배우자가 재감염의 감염원일 가능성이 낮다는 보고들도 있지만[28,29], 배우자 간의 전염을 포함한 가족내 감염은 존재하며[30], 이는 가족들이 공통적으로 노출되는 환경이 H. pylori 균에 오염되어 있다는 것을 시사한다[31]. 따라서 감염자를 제균 치료할 때, 다른 가족들도 감염 여부를 확인한 뒤에 함께 치료하는 것이 바람직하다[32].

이 연구의 한계점은 감염원을 확인할 수 없었다는 점이다. H. pylori 균주(strain)과 혈청형(serotype)을 분석하면 새로운 감염이 부부 간의 전염에 의한 것인지 알 수 있지만[33], 이 코호트 연구에서는 시행하지 못했다. 우리나라에는 무증상 감염자들이 많고[34], 성인은 소아와 달리 수직감염보다 수평감염이 흔해서 음식물, 애완동물, 시술 등을 통해서 전염되므로[35], 감염원을 입증하기가 어렵다는 제한점도 고려해야 할 것이다.

결론적으로, 감염자인 배우자를 둔 사람은 H. pylori 검사에서 음성으로 나와도 과거감염자일 가능성이 높으며, 감염이 없는 배우자를 둔 사람에 비해서 위종양이 발생할 확률이 높다. 따라서 부부 중 한 명이 감염자라면 배우자의 과거감염 여부도 고려해서 접근할 것을 권한다.

Study flow and outcomes. H. pylori-seronegative subjects who underwent gastric cancer screening with their spouses were included. Subjects were excluded if they did not undergo follow-up screening with their spouse. Among 246 seronegative subjects with an infected spouse, 92 underwent follow-up tests and were included in the case group. Among 278 seronegative subjects with a seronegative spouse, 94 underwent follow-up tests and were included in the control group. At the follow-up tests, three of the 92 cases (3.2%) and two of the 94 controls (2.1%) showed new infection (P=0.681). Five gastric neoplasms were newly detected in two cases and three spouses (5/184, 2.7%) among the 92 cases and 92 spouses, whereas none (0%) were detected among the 94 controls and 94 spouses (P=0.029).
