혼합 치료가 Helicobacter pylori의 일차 제균 요법의 대안이 될 수 있는가?

국내에서 Helicobacter pylori (H. pylori)에 대한 제균 치료는 1998년 대한 Helicobacter pylori 연구회에서 양성자펌프억제제(proton pump inhibitor, PPI), clarithromycin, amoxicillin을 포함한 3제요법을 일차 제균 치료로 권고한 이후로, 2013년 개정된 임상진료지침에서도 표준3제요법을 일차 치료로 권고하고 있다[1,2]. 그러나 표준3제요법의 제균율은 최근 10년 동안 점진적으로 감소되고 있고, 이는 항생제에 대한 H. pylori의 내성 증가에 기인한다[3]. H. pylori에 대한 효과적인 제균 요법으로 인정받기 위해서는 제균 성공률이 intention-to-treat (ITT) 분석에서 80% 이상, per protocol (PP)분석에서 90% 이상 되어야 한다[4]. 그러나 최근에 시행된 국내 다기관 연구들을 보면 현재 권고되고 있는 표준3제요법은 이러한 기준을 만족시키지 못하고 있다. 2010년부터 2015년까지 전국적인 온라인 레지스트리 데이터베이스를 구축하여 국내 H. pylori 제균 치료 현황을 분석한 연구에서 일차 치료로 7일간의 표준3제요법을 시행한 경우 제균율은 71.7% (3,859/5,383)였다[5]. 2015년부터 2017년까지 시행된 다기관 무작위 배정 연구에서 7일과 14일 동안 투여한 표준3제요법의 제균율은 PP분석에서 각각 78.5%, 78.6%로 낮게 보고되었다[6]. 2016년부터 2018년까지 전국 15개 기관이 참여한 무작위 연구에서도 7일 투여 3제요법의 제균율은 ITT분석 63.9%, PP분석 71.4%로 매우 낮은 제균율을 보였다[7]. 표준3제요법의 제균율에 가장 중요한 영향을 미치는 인자는 clarithromycin에 대한 내성 유무인데, 경기 지역의 단일 기관 연구를 보면 clarithromycin 내성률은 2003~2005년 21.2%에서 2017~2018년에는 45.9%로 급격히 증가되었다[8]. 전국적으로 15개 병원이 참여하여 2017년부터 2018년까지 이루어진 전향적 연구에서도 clarithromycin 내성률은 17.8%였다[9]. 따라서 국내에서 H. pylori에 대한 표준3제요법의 제균율 감소는 clarithromycin 내성률의 증가와 연관되어 있다고 할 수 있다. 최근 개정된 유럽과 미국의 가이드라인은 clarithromycin에 대한 내성률이 15% 이상인 경우 clarithromycin 포함 3제요법을 일차 치료제로 권고하지 않는다[10,11]. 따라서 우리나라에서도 효과적인 제균 요법의 기준에 부합하기 위한 새로운 일차 치료법의 개발이 필요하다. 항생제 내성의 증가로 제균율이 감소되고 있는 점을 고려한다면 항생제 감수성 결과에 따른 개인별 맞춤 치료가 새로운 대안이 될 수 있다. 실제 clarithromycin 내성 돌연변이를 확인할 수 있는 polymerase chain reaction 검사로 맞춤 치료를 시행한 국내 연구에서 표준3제요법과 비교하여 높은 제균 성공률을 보였다[12]. 그러나 clarithromycin 내성이 확인된 환자에서 일차 치료 약제에 대한 국내 지침이 정립되어 있지 않고, 항생제감수성검사를 위해 조직검사를 통한 검체가 필요하고 비용적 측면도 고려해야 하는 제한점이 있다. Bismuth 포함 4제요법은 일차 치료에 실패한 경우 이차 약제로 권고되어 왔으나, clarithromycin 내성이 높은 지역에서는 일차 치료로 권고되고 있다[2]. 그러나 복용 방법이 복잡하고, 약제의 부작용이 비교적 흔하여 더 효과적인 일차 제균 요법이 필요한 실정이다. 최근 새로운 제균 요법들이 대안적으로 제시되었는데, 대표적으로 순차 치료(sequential therapy), 동시 치료(concomitant therapy), 혼합 치료(hybrid therapy)가 있다. 순차 치료는 PPI와 amoxicillin을 5~7일간 투여 후 PPI, clarithromycin, metronidazole을 5~7일간 투여하는 방식이다. 2000년 이탈리아에서 처음 소개되었을 때 10일간의 순차 치료로 ITT 기준 98%의 높은 제균율을 보였다[13]. 하지만 국내 연구 결과는 외국의 보고와는 달리 순차 치료의 제균율이 높지 않았다[14,15]. 대안적으로 제시된 제균 치료들의 최근 제균율을 알아보기 위해 2015년부터 2020년까지 발표된 국내 H. pylori 제균 치료 연구를 PubMed를 통해 검색하였고, 그중 일차 치료로 순차 치료, 동시 치료, 혼합 치료를 시행한 무작위 대조군 연구를 분석하였다(Table 1). 순차 치료의 치료 기간은 10일 투여와 14일 투여를 비교한 논문을 제외하고 8개 연구에서 투여 기간은 10일이었다. 순차 치료의 제균율은 ITT분석에서 58.0~82.4%, PP분석에서 70.0~91.4%로 대부분의 연구에서 효과적인 제균요법의 기준에 미달하였다. Clarithromycin에 대한 내성은 순차 치료의 제균율을 감소시키는 것으로 알려져 있고[16], metronidazole에도 동시 내성이 있는 경우 효과가 크게 감소하기 때문에, 국내에서 순차 치료의 낮은 제균율은 clarithromycin의 내성 증가와 연관되어 있다. 또한 국내에서 clarithromycin에 내성이 있는 경우 53.2%에서 metronidazole 또는 amoxicillin에 다제 내성을 보이는 것으로 나타났다[9]. 따라서 높은 빈도의 다제 내성 역시 순차 치료의 낮은 제균율과 연관되어 있을 것이다. 동시 치료는 PPI, amoxicillin, clarithromycin, metronidazole의 4가지 약제를 동시에 투약하는 방법이다. Table 1에서 동시 치료는 4가지의 약제를 7~14일간 투여하였으며, 대부분 연구에서 투여 기간은 10일이었다. 동시 치료의 제균율은 ITT분석에서 75.6~83.04%, PP분석에서 89.8~98.5%로 대부분의 동시 치료 성적은 PP분석에서 효과적인 제균 요법의 기준을 만족시켰다. Clarithromycin 내성 균주만을 대상으로 한 국내 무작위 배정 연구에서 동시 치료의 제균율이 순차 치료에 비하여 유의하게 높았다(93.3% vs. 53.9%) [16]. Metronidazole은 투여 횟수, 양, 기간을 늘림으로써 내성을 극복할 수 있다고 알려져 있는데[17], 동시 치료에서 순차 치료에 비해 metronidazole 등의 항생제 투여 기간이 늘어나기 때문에 제균율이 향상되었을 것이다. 개정된 Maastricht V/Florence 진료지침에서도 동시 치료를 시행하는 경우 14일 동안 투여하도록 권고하고 있으며, clarithromycin 내성률이 15% 이상인 경우 순차 치료는 권고하고 있지 않다. 혼합 치료는 순차 치료와 동시 치료를 병합한 형태의 치료법으로 초기 5~7일간만 투약되는 amoxicillin을 전체 투약 기간으로 늘린 치료법이다. 2015년부터 1개의 무작위 배정 연구가 있었으며, 치료 기간은 10일이었으며 제균율은 ITT분석에서 78.8%, PP분석에서 89.6%로 효과적인 제균 요법의 기준에는 미달하였지만, 동시 치료와 비슷한 제균율을 보였다. 하지만 혼합 요법에 대한 연구가 아직 많지 않아 정확한 제균율과 일차 치료로서의 가능성을 판단하기는 어렵다.

Helicobacter pylori Eradication Rates according to the Regimens among the Randomized Korean Trials Published from 2015 to 2020

이번 호에서 Park 등[18]은 일차 제균 치료로 시행된 10일 혼합 치료와 10일 동시 치료의 제균율을 후향적으로 비교분석하였다. 각 군에 62명이 포함되어 총 124명의 H. pylori 감염 환자들이 분석되었다. 혼합 치료와 동시 치료의 H. pylori 제균율은 ITT분석에서 각각 74.2% (46/66명)와 67.7% (42/62명)였고(P=0.429), PP분석에서 각각 88.5% (46/52명)와 82.4% (42/51명)로(P=0.380), ITT분석과 PP분석 모두에서 두 군 간의 H. pylori 제균율은 유의한 차이가 없었다. 순응도는 혼합 치료군에서 91.4%, 동시 치료군에서 95.7%로 양 군 간 유의한 차이가 없었다(P=0.717). 일반적으로 혼합 치료가 clarithromycin, metronidazole의 투약 기간이 절반이기 때문에 동시 치료보다 약제 부작용이 낮을 것으로 기대되지만, 이번 연구에서 부작용은 각각 75.0%, 70.6%로 매우 높게 보고되었으나 두 군 간의 차이는 보이지 않았다. 이번 연구는 비록 후향적 연구라는 제한점이 있지만 연구 시작 시점에서 순차적으로 환자를 선정하여 환자 선택 비뚤림을 줄이고자 하였다. 혼합 치료의 제균율은 동시 치료와 비슷하였으나, 효과적인 제균 요법의 기준에는 ITT기준과 PP기준 모두 미달되었다. 통계적 유의성은 없었으나 10일 혼합 치료가 10일 동시 치료보다 제균율이 더 높았던 점은 긍정적으로 여겨지지만, 10일 투여 혼합 치료를 H. pylori 일차 제균 치료의 대안으로 사용하기에는 아직 근거가 부족해 보인다. 그러나 국내 네트워크 메타분석에서 10일 이상의 혼합 치료가 표준3제요법에 비해 유의하게 높은 제균율을 보였고, 동시 치료와 비슷한 수준의 제균율을 보인 점은 매우 고무적이다. 유럽의 가이드라인에서 clarithromycin 내성률이 15% 이상이면, 더 이상 clarithromycin 포함 3제요법은 권고하지 않고 bismuth 포함 4제요법이나 동시 치료를 권고하고 있다. 혼합 치료는 아직 근거가 부족하고 항생제 내성 정도에 따라 지역별로 다양한 제균율을 보이기 때문에 bismuth 비포함 4제요법으로는 혼합 치료보다 동시 치료를 우선적으로 권고하고 있다[11]. 그러나 이번 연구와 국내 네트워크 메타분석처럼 혼합 치료가 동시 치료와 비슷한 제균율을 보인다면 일차 제균 치료의 대안이 될 가능성은 있다. 또한 동시 치료에서 치료 기간을 14일로 연장한 경우 제균율의 향상이 있었던 것처럼, 혼합요법도 14일로 연장 투여한다면 제균율이 더 향상될 수 있을 것으로 기대된다.

요약하면 이번 연구에서 혼합 치료의 제균율이 효과적인 제균 요법의 기준에는 미달하였으나, 통계적 범위 내에서 동시 치료에 비해 더 나은 제균율을 보인 점은 주목할 만하다. 국내 clarithromycin 내성률이 15% 이상이고 표준3제요법의 제균율이 매우 낮은 현실에서 새로운 일차 제균 요법이 절실하다. Bismuth 포함 4제요법, 동시 치료와 더불어 혼합 치료도 대안적인 방법으로 충분한 가능성을 보여주고 있고, 향후 투여 기간의 연장을 포함한 다양한 연구가 필요하다.
