Trans Abstract

Peptic ulcer disease involves breakdown of the epithelial lining of the stomach or duodenum. Stenosis and associated gastric outlet obstruction is the least frequent complication of peptic ulcer disease. Most cases of stenosis occur because of duodenal or pyloric cannel ulceration. Although etiologies of peptic ulcer disease and the affected patient populations have changed, the most common etiologic factors remain Helicobacter pylori infection and use of non-steroidal anti-inflammatory drugs. In recent years, eradication of H. pylori and the use of proton pump inhibitors have reduced the incidence of complications. Because the predominant cause of gastric outlet obstruction has increasingly been attributed to malignant cancer in recent years, endoscopic examination is important to differentiate benign peptic stenosis from malignant stenosis. Medical treatment such as acid-reducing drugs or H. pylori eradication is the mainstay of treatment. Endoscopic dilation or surgical management may be considered in cases of refractory or recurrent stenosis despite medical treatment. Among the major complications of peptic ulcer disease, this report will provide an overview of peptic ulcer-related stenosis.

서 론

소화성 궤양은 위산과 펩신의 공격으로 인한 위 및 십이지장 점막의 결손을 일컫는데, 일반적으로 위산 역류로 인해 발생하는 역류성 식도염에 의한 식도 궤양을 제외한 위와 십이지장에 발생한 궤양을 일컫는다[1]. 최근 Helicobacter pylori의 제균 치료 및 강력한 위산분비억제제(양성자펌프억제제)가 사용되면서 소화성 궤양의 합병증은 전 세계적으로 감소하고 있다[2]. 소화성 궤양의 합병증은 출혈, 천공, 협착 등이 있으며, 이 중 협착이 가장 드물지만 임상적으로 문제가 된다. 특히, 소화성 궤양에 의한 협착은 위배출구 폐색을 유발할 수 있는 중요 질환 중 하나이다. 이번 종설에서는 소화성 궤양의 중요 합병증 중 협착, 특히 유문륜 및 십이지장 협착에 대해 알아보고자 한다.

본 론

1. 역학

최근 선진국의 경우 양성자펌프억제제 사용 및 H. pylori 제균 치료의 효과로 소화성 궤양의 발생은 전체적으로 감소하고 있다. 하지만 인구가 고령화되면서 소화성 궤양 합병증으로 인한 입원은 감소하고 있지 않다[3]. 국내의 경우는 H. pylori의 감염 유병률은 감소하고 있지만, 인구가 초고령화되면서 궤양을 유발하는 약제(특히 비스테로이드성 진통소염제와 저용량 아스피린) 사용이 증가하면서 소화성 궤양의 발생은 큰 변화가 없다[2,4,5]. 현재까지의 소화성 궤양의 합병증 보고는 대부분 출혈과 천공에 대한 내용이다. 반면, 협착의 경우는 협착을 정의하는 기준이 다르고, 대부분 증상이 있어 내시경을 시행한 환자이거나 입원 환자들을 대상으로 하고 있어 발생률을 정확하게 파악하기 어렵다. 국내의 소화성 궤양에 대한 보고 대부분이 검진 내시경을 시행한 사람을 대상으로 하고 있어 소화성 궤양 협착에 대한 유병률은 국내 자료에서 찾아보기 힘들다[6-8]. 앞서 말한 제한점이 있지만 소화성 궤양에 대한 서구 보고에 따르면 지역에 따라 차이가 있지만, 협착에 의한 폐색은 1∼3% 정도이다[9-11]. 최근의 보고 모두 증례 수가 많지 않아 남녀 차이는 알 수 없으며, 평균 연령은 50∼70대 사이였다[12-16]. 오래 전에 발표된 연구에서는 소화성 궤양 합병증의 8∼56%가 협착으로 인한 것으로 보고하고 있다[17,18]. 이러한 발생의 차이는 H. pylori의 감염률 및 비스테로이드성 진통소염제의 사용률의 차이와 연관이 있을 것으로 생각된다.

2. 병태생리

정확하게 알려져 있지 않지만, 소화성 궤양 협착을 악화시키는 인자는 크게 가역적 원인과 비가역적 원인으로 나눌 수 있다. 가역적인 원인으로 급성 궤양과 연관된 경련, 부종, 염증 및 유문부의 운동 장애가 있다. 비가역적이거나 회복이 되더라도 느린 회복속도를 보일 수 있는 원인으로 만성적인 섬유화와 반흔으로 인한 유문륜 및 십이지장의 구조 변형이 있다. 합병증 발생의 위험이 높은 궤양은 불응성 궤양, 크기가 20 mm 이상, 유문륜 주위에 발생한 경우라고 알려져 있다[19].

합병증을 동반한 소화성 궤양의 가장 중요한 원인은 H. pylori 감염과 비스테로이드성 진통소염제의 사용이다[1]. 만약 H. pylori 감염이나 비스테로이드성 진통소염제의 사용이 확실히 배제되었다면, 졸링거-엘리슨 증후군(Zollinger-Ellison syndrome)과 같은 위산분비 과다가 될 수 있는 질환이나 크론씨병이나 베체트병과 같은 만성 염증성 질환을 배제할 필요가 있다.

3. 증상

소화성 궤양의 협착은 급성 궤양으로 인한 부종성 변화 및 만성 반흔에 의해 발생하며, 유문륜이나 십이지장에서 발생한다[20]. 소화성 궤양 협착이 있더라도 배출구 장애의 정도, 환자의 나이, 동반 질환 및 전반적인 건강 상태에 따라서 증상은 다양하게 나타날 수 있다. 협착이 심하지 않은 시기에는 소화성 궤양으로 인해 소화불량이나 상복부 통증을 주로 호소하며, 폐색의 증상이 간헐적으로 일어나는 시기부터는 오심, 소화불량, 복통이 반복되고, 폐색이 더욱 악화되면 조기 포만, 오심, 구토, 식후 복통 및 체중 감소 등을 호소할 수 있다[20,21]. 수술이나 내시경 풍선확장술을 받은 환자들이 호소하는 가장 흔한 증상은 구토이며, 하루에 한 번 이상 구토를 하는 환자가 74% 정도이다[22]. 폐색으로 인한 증상은 통상적으로 병원 방문 한 달 전부터 발생하지만, 1/3의 환자에서는 증상이 3달 전부터 지속되는 경우도 있다[22].

4. 진단

소화성 궤양 병력이 있는 환자가 조기 포만, 오심, 구토, 식후 복통 및 체중 감소 등을 호소하면 소화성 궤양 협착에 의한 위배출구 장애를 의심해야 한다. 식사를 하거나 음료를 마시고 3시간이 지난 후 복부 진찰에서 진탕음(succussion splash)이 확인되면 위배출구 장애 진단의 단서가 될 수 있다[23]. 감별진단을 위해 필요한 가장 중요한 검사는 상부위장관 내시경이다. 1970년대까지 위배출구 장애의 가장 흔한 원인은 소화성 궤양이었다. 최근에는 강력한 위산분비억제제의 사용, H. pylori균의 치료, 비스테로이드성 진통소염제의 위험성 인식으로 소화성 궤양 협착의 빈도가 감소하여, 위배출구 장애의 가장 흔한 원인은 악성 질환이다[24,25]. 따라서 위배출구 장애 증상이 있는 환자에서 악성 질환에 의한 협착을 배제하는 것이 중요하다. 8∼12시간 금식 후 시행한 내시경검사에서 위내 음식물이 남아 있거나 위액의 저류가 관찰되고, 내시경(직경 9∼10 mm)이 십이지장을 통과할 수 없는 경우 협착에 의한 위배출구 장애라고 진단할 수 있다(Fig. 1A, B, Fig. 2A, B) [25]. 소화성 궤양 협착으로 위배출구 장애 증상이 있는 환자들 중 1/3에서는 활동성 궤양이 동반되어 있다[21]. 내시경검사의 장점은 이러한 활동성 궤양이 있거나 악성 질환이 의심되는 경우 조직검사를 시행할 수 있고, 필요하다면 H. pylori 감염 여부를 평가하기 위한 급속요소분해검사를 시행할 수 있다. 단순 복부 X선 촬영에서 위내 액체의 저류를 확인하면 위배출구 장애 진단에 도움이 된다(Fig. 1C). 복부 컴퓨터단층촬영은 위벽의 비후, 임파선 종대 및 주위 구조물(췌장, 담관, 후복강 등)의 이상 여부를 확인할 수 있어 악성 질환 등을 감별하는 데 도움이 된다(Fig. 1D). 위장조영술의 경우에는 협착의 부위 및 정도를 평가할 수도 있다(Fig. 1E).

5. 치료

소화성 궤양의 약물 치료는 위산분비억제제 사용과 H. pylori 제균 치료가 근간이다. 소화성 궤양 협착에 의한 폐색 증상이 없는 환자에서는 경구 양성자펌프억제제를 사용하여 활동성 궤양을 치료하고, H. pylori가 확인되면 제균 치료를 시행하여 궤양의 재발 위험을 감소시킨다[26,27]. 비스테로이드성 진통소염제가 원인이라면 중지하거나 선택적 cyclooxygenase-2 억제제로 변경한다[4,28]. 만약 저용량 아스피린과 같은 항혈소판제를 중단할 수 없으면서 소화성 궤양 합병증의 발생 위험이 높은 환자라면, 합병증 발생을 예방하기 위한 목적으로 지속적인 양성자펌프억제제 사용이 필요하다[4,29,30].

소화성 궤양 협착으로 인한 구토 및 섭취 장애가 있는 경우 탈수, 전해질 불균형 및 영양 결핍이 발생할 수 있다. 만성적인 구토 증상이 있는 경우 적절한 수액 및 영양의 공급과 전해질 장애 교정이 필요하다. 완전 폐색이 발생한 경우 비위관 삽입을 통한 위감압 치료를 시행한다. 급성 소화성 궤양의 급성 염증으로 인한 폐색 때문에 경구 섭취가 제한되는 경우에 48∼72시간 정도 위산분비억제제를 정맥 투약하면, 급성 부종 및 염증이 호전되면서 증상 호전을 기대할 수 있다[24]. 한 보고에 의하면, 소화성 궤양에 의한 위배출구 장애로 약물 치료를 받은 87명의 환자 중 38명(44%)의 환자에서 약물 치료만으로 치료가 가능하였다[31]. 협착이 반흔에 의한 비가역적인 단계로 진행하여, 약물 치료에 반응이 없다면 수술[31]이나 내시경 풍선확장술[12,13,15,16,32]이 필요하다.

내시경 풍선확장술에 사용되는 기구는 보통 길이 55∼80 mm, 직경 6∼20 mm의 풍선이다(Fig. 2D). 통상 유도 철선을 사용하여 풍선확장술을 시행하며, 대개 한번으로 끝나지 않고 수차례 반복이 필요하다. 풍선확장술을 하더라도 지속적인 위산분비억제제 투약 및 H. pylori 감염 확인 및 제균 치료는 필요하다[14]. 풍선확장술은 협착이 국소적이면서 길이가 짧을 때 보다 효과적이라고 알려져 있다[33]. 통상 15 mm까지 확장이 가능할 때까지 1∼3주 간격으로 시행한다[12,24,34]. 유문륜의 양성 협착에 대한 풍선확장술은 대부분이 소규모로 이루어진 후향적인 연구들로, 55∼100%의 환자에서 풍선확장술 후 추가적인 수술적 치료가 필요하지 않았다[12,13,15,16,32]. 풍선확장술의 중요 합병증은 천공으로, 0∼7.4% 정도에서 발생한다[12,13,15,16,32]. 대부분의 천공은 18 mm 이상 확장술을 시행하였을 때 발생한다(Table 1) [12,13,15,16,21,32]. 풍선확장술에 불응하는 양성 협착의 내시경 치료로 단기간 자가 확장성 금속 스텐트 유치를 시도한 보고도 있지만[35,36], 아직까지는 이에 대한 근거 자료가 부족하다.

Clinical Outcomes of Endoscopic Balloon Dilation for Benign Pyloric Stenosis

H. pylori가 발견되지 않고, 강력한 위산분비억제제가 개발되지 않았던 시대에는 위관련 수술의 가장 흔한 원인이 소화성 궤양에 의한 협착이었지만[37], 최근에는 기본적인 약물 치료와 내시경 확장술에 반응하지 않는 경우와 내시경 확장술을 시행할 수 없는 경우에 국한하여 수술을 시행하고 있다. 소화성 궤양의 협착에 대한 수술은 위산분비 억제 관련 수술(미주신경절단술[vagotomy], 위전정부절제술[antrectomy], 위원위부절제술[distal gastrectomy])과 위배액 수술(위공장연결술[gastrojejunostom]과 유문성형술[pyloplasty])을 시행하며(Fig. 1F), 전반적인 환자 전신 상태에 따라 수술 방법을 결정한다[9]. 통상적으로 반흔으로 인한 해부학적 변형 때문에 위공장연결술이 유문성형술보다 선호된다. 폐색 부위를 절제하는 위부분 절제술(위전정부절제술 또는 위원위부절제술)과 미주신경절단술을 같이 시행하기도 한다. 소화성 궤양 협착으로 수술을 시행한 90명 환자를 대상으로 시행된 전향적 무작위 임상 시험에서 미주신경절단수술+위공장연결술, 미주신경절단수술+유문성형술, 미주신경절단수술+위전정부절제술을 비교시, 미주신경절단수술+위공장연결술이 다른 수술법에 비해 합병증이 적었고, 수술 후 식이를 시작하는 시기가 평균 4일으로 우수하였다[38].

6. 재발의 예방

경구 섭취가 가능해지면, 위산분비억제제를 주사제에서 경구제로 변경한다. 재발 예방을 위해서는, 소화성 궤양 협착의 원인(특히, H. pylori 및 비스테로이드성 진통소염제)을 파악하고 제거하는 것이 중요하다[20]. 궤양 협착과 같은 소화성 궤양 합병증의 고위험군에서 지속적인 저용량 아스피린이나 비스테로이드성 진통소염제 사용이 필요한 경우 위산분비억제제의 동반 사용이 필요하다[4,39]. 하지만 실제 임상에서는 환자가 저용량 아스피린이나 비스테로이드성 진통소염제의 사용을 의료진에게 말하지 않거나, 의료진이 소화성 궤양의 위험 인자 동반 여부에 대한 평가를 시행하지 않는 경우가 많다. 한 보고에 의하면 소화성 궤양 합병증의 병력이 있는 환자 진료 시, 이전 소화성 궤양의 합병증의 여부를 확인하지 않는 경우가 92%였다[40]. 합병증을 동반한 소화성 궤양 환자가 예방적인 위산분비억제제 복용 없이 비스테로이드성 진통소염제를 사용하게 되면, 수술적 치료를 받았더라도 향후 소화성 궤양의 재발 위험이 높아지므로, 의사 및 환자 모두 이에 대한 주의가 필요하다[41]. 만약 H. pylori 감염 및 비스테로이드성 진통소염제의 사용력이 없는 소화성 궤양이라면, 궤양의 다른 원인을 찾기 위한 노력이 반드시 이루어져야 한다[42,43].

결 론

H. pylori에 대한 치료 및 강력한 위산분비억제제의 사용 이후 합병증을 동반한 소화성 궤양은 감소하고 있다. 하지만 반복적인 궤양의 발생과 치료로 인한 구조적 변형이나 협착 부위의 급성 궤양으로 인한 점막 부종 등으로 위배출구 장애가 발생할 수 있다. 이러한 경우 원인이 되는 약제(특히, 비스테로이드성 진통소염제 또는 아스피린)가 있다면 중단 또는 변경이 필요하며, 강력한 위산분비억제제를 사용하여 급성 궤양 및 이로 인한 부종을 치료한다. 반흔으로 인한 협착이 있는 경우에는 내시경 풍선확장술을 우선 고려한다. 내시경 풍선확장술을 실패한 경우 또는 시행을 할 수 없는 환자에서는 수술적 치료를 고려해야 한다.
