Association of Helicobacter pylori Infection, Polymorphism of Interleukin-1B and Interleukin-1RN with Gastric Cancer in Korea

Abstract
Background/Aims: Helicobacter pylori (H. pylori) infection is an established risk factor of gastric cancer but cofactors involved in producing malignant transformation of gastric mucosa are currently unknown. Individual variations in cancer risk have been associated with specific variant alleles of different genes and recent results suggest that single nucleotide polymorphism of interleukin-1 beta(IL-1β) and interleukin-1 receptor antagonist(IL-1ra) has an important role of tumorigenesis in gastric cancer. According to recent study, carriers of IL-1B-511 T and IL-1RN*2 had an increased risk of developing gastric cancer. However, the results have not been concordant from study to study which means ethnic differences as well as genetic heterogeneity should be taken into consideration in studies of any candidate gene. We investigate genotype and allele frequencies of IL-1B and IL-1RN gene polymorphisms in Korean population of gastric cancer patients and healthy control, and assess whether these two polymorphic genes are involved in the susceptibility to gastric cancer. Methods: Blood samples(buffy coat) have been collected from patients with gastric cancer and healthy control. DNA was extrated from buffy coat using a commercially available kit. The polymorphisms at positions -511 and -31 of IL-1B gene were genotyped by polymerase chain reaction(PCR)-restriction fragment length polymorphism(RFLP), and the penta-allelic VNTR polymorphism in intron 2 of the IL-1RN gene was genotyped by PCR. Results: Both in patient and control group, it was found that IL-1B polymorphisms at -31 and -511 had near complete linkage disequilibrium. No difference in genotype of IL-1B and IL-1RN polymorphism was found between patient and control group(p>0.05). The genotypes of IL-1B gene in patient group and control group were as follows; in patient group, 24.4% for C/C type, 48.9% for C/T type, 26.7% for T/T type and in control group, 13.3%, 62.2%, 24.4% respectively. For IL-1RN gene, the genotypes were as follows; 91.1% for L/L, 6.7% for L/*2, 2.2% for *2/*2 in patient group and 84.4% for L/L, 15.6% for L/*2, and 0% for *2/*2 type in control group. Also, there was no difference in genotype of IL-1B and IL-1RN polymorphism between H. pylori positive and negative group as well as intestinal and diffuse type of gastric cancer. Conclusions: IL-1B-511 and IL-1RN genetic polymorphisms were not associated with gastric cancer in a multistep carcinogenesis model. This result suggests that ethnic differences as well as genetic heterogeneity should be taken into consideration in studies of host susceptibility to gastric cancer.
