역류성 식도염과 소화성 궤양의 성별 차이–성호르몬 측면에서
소아는 어른의 축소판이 아니라는 사실에 대해 모든 의사가 동의한지는 오래 되었지만 ‘여자는 작은 남자가 아니다’라고 인식된 것은 얼마 되지 않았다. 최근 주목을 받고 있는 성차의학 또는 젠더의학은 성과 젠더, 혹은 그 둘의 상관관계에서 발생하는 임상적 질환을 연구하는 것으로 ‘성별 특성을 반영하는 의학 연구’라 할 수 있다. 질환의 발생률, 증상, 병적상태(morbidity), 사망률에 있어 남녀 차이가 있는 경우가 많고[1] 같은 결과가 나오지 않는 일관성의 결여가 성별 특성을 감안하지 않은 연구 방법에서 기인할 수 있음이 알려지면서[2,3] 연구비를 관리하는 Canadian Institutes of Health Research (2010), European Commission (2014), US National Institutes of Health (2016), German Research Foundation (2020) 등 주요 기관에서 sex and gender analysis의 중요성을 강조하고 있다. 남녀 차이가 없다는 전제하에 남녀 각각의 데이터를 합쳐 분석하는 것이 당연시 되었지만 이러한 남녀 데이터의 pooling으로 인해 연구 결과가 오히려 의미 없어질 수 있음이 지적되고 있는 것이다[3]. 이러한 자각은 발표 논문의 질을 높이기 위한 peer-review journals에서의 편집 가이드라인(editorial guidelines)에 반영되어[4] 남녀 분석을 진행하지 않고 논문을 접수했을 때는 편집자(editor)가 다시 수정하여 접수할 것을 권유하는 학회지가 늘어나고 있다.
소화기 질환에서도 성차가 분명한 질환들이 많다. 예를 들면 기능성 소화불량증, 과민성장증후군 등 기능성 위장관장애(functional gastrointestinal disorder, FGID)는 여자에게 흔한 반면 소화기암은 대부분 남자에서 자주 발생한다. 그 이유에 대해서는 점차 밝혀지고 있는 바 FGID는 그 병인에 스트레스가 중요한데 여자가 이러한 스트레스에 취약하며 내장과감각이나 위장관 운동(motility), 뇌ㆍ장ㆍ축(brain-gut-axis) 기전에 있어 남녀 차이가 많다고 알려지고 있다. 이에 더하여 위, 식도, 대장의 기능성 질환이 동시에 발생하는 FGID 중복증후군의 발생은 여자에서 빈번한데[5] 이는 어쩌면 당연한 결과인 것 같다. 이에 반해 소화기암 즉 식도암, 위암, 대장암은 남성에서 여성보다 2배 정도 흔한데 남자에서 흔한 흡연, 음주, 식이의 차이 외에 암 발생을 억제하는 여성호르몬의 역할이 주목받고 있다[6,7]. 또한 B형 간염이나 C형 간염 후 진행되는 간섬유화가 여성에서는 비교적 경미하지만 남성에서 흔하고 간암의 유병률 또한 남성에서 매우 높은데 이들 여러 질환의 진행 지연에 있어 에스트로젠의 역할이 주목받고 있다[8]. 에스트로젠은 스테로이드 계열로서, 인간 생식기관의 발달과 성장을 조절하는 물질로 처음 발견되었고, 에스트로젠 수용체(estrogen, ER)는 생식계 이외의 심혈관, 골격 및 신경 내분비계의 다른 생리적, 병리학적 과정에 관여하는 것으로 밝혀지고 있다. 에스트로젠에 대한 연구는 1996년에 서로 다른 두 종류의 에스트로젠 수용체인 ERα와 ERβ가 발견되면서 새로운 국면을 맞이하게 되었고 좀 더 넓은 범위에서의 다양한 연구가 이루어질 수 있게 되었다. 남성에 호발하는 대장암의 경우 ERβ가 대장암 발생을 막아 여성에서는 상대적으로 그 발생률이 낮다가[9,10] 폐경이 되면 그 발생 빈도가 높아진다는 것이 밝혀졌다. 이에 반해 남성호르몬인 안드로겐의 수용체(androgen receptor, AR)가 생식기관 이외 조직의 발암에 여러 경로로 관여하고 있음이 밝혀지고 있는데 예를 들어 AR의 과발현은 발암을 촉진시키고 림프절 전이 및 불량한 예후와 연관된다는 것이 보고되었으며[11,12] 위의 신생물 조직을 이용한 연구에서도 AR 양성 반응을 보인 환자에서 독립적으로 낮은 생존율이 발견되었다[11,13,14]. 한편 약 2:1 정도로 남성에서 위암이 호발하는 경향은 전 세계적으로 동일하게 나타나고 있는 현상이지만[15], 모든 연령대에서 남성이 항상 우세한 것은 아니었다[16]. 즉, 40대 이하에서는 오히려 여성에서 더 높고 미분화형이 대다수인 결과로 미루어 볼 때 여성호르몬은 장형 위암은 억제하지만 미만형은 촉진하는 등 위암 조직형에 따라 다른 역할을 할 가능성이 시사되고 있는 것이다[17]. 이러한 식도, 위 대장의 운동 및 암 그리고 간, 췌장암과 뇌에 영향을 미치는 호르몬의 영향을 정리하면 Table 1과 같다[6-14,18-22].
본고에서는 소화기 질환 중 여자보다 남자에서 흔히 발생하는 역류성 식도염과 소화성 궤양에서의 성별 차이를 간략히 살펴보고자 한다. 역류성 식도염의 발생 빈도가 남자에서 높은데 반해 여자에서는 낮다가 폐경 후 그 발생 빈도가 올라가면서 질환의 정도 또한 심해져 에스트로젠이 역류성 식도염의 방어인자로 해석되고 있다[23,24]. 이러한 사실은 건강검진자 약 10,000명을 대상으로 폐경과 위식도 역류 질환(역류성 식도염과 비미란성 역류 질환 포함)의 관련성에 대해 조사한 연구에서도 뒷받침되는데 폐경 후 시간이 지날수록 체질량지수(BMI), 증상을 동반한 위식도 역류 질환이 증가하다가 폐경 후 20~29년이 지나면서 역류성 식도염이 급격히 증가한 보고에서도 확인된 바 있다[25]. 역류성 식도염이 남자에 많은 이유는 하부식도조임근 등의 구조적 인자와 함께 식도점막세포의 방어에 관련되는 에스트로젠의 항염증 작용, 밀착연접 관련 단백질 발현이 낮은 것도 그 원인으로 뽑히고 있다[26]. 이러한 에스트로젠의 항염증 역할에 대해서는 주로 동물모델에서 제시되고 있는데 만성 역류성 식도염 마우스모델에서 에스트로젠이 식도 점막을 보호하였고 외부에서 가해진 nitric oxide가 주로 수컷 쥐에서 중성구와 림프구 침윤을 일으키면서 식도 점막을 손상시킨 반면, 암컷 쥐에서는 경미한 손상만 유발됨이 보고되었다[27]. 또한 난소제거술을 받은 암컷 쥐에서는 식도 염증이 더 심했는데 수컷 쥐와 난소제거술을 받은 암컷 쥐에 17β-estradiol을 주입한 결과 비만세포(mast cell)에 의한 세포 독성과 tumor necrosis factor-α를 비롯한 사이토카인의 분비가 감소하면서 식도 손상이 감소함을 보여주었다[27]. 하지만 17α-estradiol을 주입했을 경우에는 전혀 보호 작용이 발동하지 않아 에스트로젠 중에서도 17β-estradiol이 중요함을 알 수 있었다[27]. 또한 토끼 동물실험에서 17β-estradiol을 2주 주입한 후 식도를 분리하여 ussing chamber 실험을 진행한 결과 17β-estradiol 치료군은 occludin의 증강을 통해 위산과 같은 자극원에 대한 세포간격 증가가 감소했음이 밝혀졌다[28]. 이에 더하여 에스트로젠은 식도 대식세포이동저해인자(macrophage migrating inhibitory factor, MIF)를 억제하였는데[29] 에스트로젠이 상처 치유에 있어 MIF를 억제함이 알려지면서[30] 에스트로젠에 의한 항염증 작용 및 밀착연접 관련 단백질 발현 증가 그리고 MIF 억제가 여자에서의 역류성 식도염 유병률 감소를 설명하는 주된 기전으로 꼽히고 있다. 이처럼 역류성 식도염의 유병률이 남자에서 여자보다 3.5배 높지만 이와는 반대로 식도 손상 없이 가슴쓰림과 산 역류 증상이 심한 비미란성 역류 질환(non-erosive reflux disease, NERD)의 경우 여자에서 남자보다 의미있게 높은 것은(Fig. 1) [31] 위산에 의한 물리적 손상보다는 NERD의 주 기전인 위산에 대해 느끼는 내장 과민성이 여자에서 더 높기 때문이다[26]. 위산 자체보다는 내장 과민성으로 인해 식도 점막이 예민해져 있는 NERD에서 양성자펌프억제제(proton pump inhibitor, PPI)에 대한 반응이 낮은데 여자에서 PPI의 효과가 남자보다 낮은 이유 또한 같은 맥락으로 해석할 수 있다[26].
2006년 전국 40개 기관에서의 25,536명 건강 검진자에서의 상부 소화성 궤양의 분포를 보면 십이지장궤양은 남자에서 7.8%로 여자의 3.5%보다 2.22배 높았고, 위궤양의 경우 남자는 5.8%로 여자의 2.7%보다 2.15배 높았다[32]. 또한 천공이나 출혈 같은 소화성 궤양 합병증 발생률이 여자에서 남자보다 낮은데(Table 2) [33-35] 그 이유는 남자에 빈번한 헬리코박터 파일로리 감염, 음주, 흡연의 차이에도 기인하겠지만 위점막세포 방어에 관련하는 에스트로젠의 항염증 작용, 밀착연접 관련 단백질 발현이 남성에서 낮기 때문으로 해석되고 있다[36]. 다만 대부분의 논문[32-35]에서는 역류성 식도염과 소화성 궤양 질환 발생의 위험인자인 흡연, 음주에 대해서는 고려되었지만 여성의 폐경 나이 이후에 대한 분석이나 복부 비만에 대한 보정이 이루어지지 않는 한계가 있다. 하지만 이들 질환에 미치는 성호르몬의 영향을 정리해볼 때[26,27,36-39] 이들 질환 발생에 성호르몬의 영향은 확실한다고 생각된다(Table 3).
위 상피는 염산(HCl), 펩시노겐/펩신, 담즙 등의 내인성 유해 인자들에 의해 지속적으로 공격받고 있고, 약물, 술 그리고 세균과 같은 외인성 물질들이 끊임없이 위 상피를 거쳐 간다. 그러나 매우 정교한 생물학적 시스템이 점막의 손상을 막아 주고 혹시 일어나는 점막 손상에 대한 복구를 가능하게 한다. 결국 점액, 점막상피세포, 혈액순환, 중탄산염 분비 등 방어인자가 건재하면 위산이나 펩신과 같은 공격인자에도 불구하고 소화성 궤양은 일어나지 않게 되는 것이다. 소화성 궤양의 발생은 물론 출혈, 천공과 같은 소화성 궤양 합병증이 남성에 비해 여성에서 낮게 발생하는 이유로 에스트로젠에 의한 세포와 세포 사이의 틈을 막아주는 밀착연접 관련 단백질 발현 증가[24]와 십이지장 중탄산염 분비(duodenal bicarbonate secretion, DBS)의 촉진이 에스트로젠에 의해 촉진됨이 인체[37]와 동물실험[38]에서 증명된 바 있다. Tuo 등[37]은 역학 자료를 분석하여 십이지장궤양의 남녀별, 연령별 발생을 알아본 후 건강인에서 십이지장 중탄산염 분비를 측정하고 ER의 발현에 대해 immunoblot과 면역조직화학 분석법(immunohistochemical analysis)을 시행하였다. 그 결과 여자에서 남자보다 십이지장궤양 발생률이 유의하게 낮았는데 특히 폐경 전 여자(20~49세)에서 같은 연령의 남자보다 3.91~5.09배 낮았으나 60세 이상 연령군에서는 여자가 남자보다 1.32배 이하로 그 발생 차이가 작아졌다[37]. 또한 20~29세 연령군의 여성에서 남성보다 기초(basal) 내지 산 자극(stimulated)에 의한 십이지장 중탄산염 분비가 높았지만 60~69세에서는 이런 남녀 차이가 없었고[37] 20~29세 여성에서 60~69세 여성보다 십이지장 중탄산염 분비가 높았으나 남성에서는 연령별 차이가 관찰되지 않았다. 이에 더하여 폐경 전 여성 혈청 estradiol 수치는 월경 주기에 따라 십이지장 중탄산염 분비와 비례 관계가 있었는데 이는 에스트로젠이 십이지장 상피세포막(plasma membrane)과 세포질에서 발현되는 ER에 결합한 후 십이지장 중탄산염 분비를 조절함으로써 여성에서의 십이지장궤양 발생의 위험성을 낮춘다는 것이다[37]. 또한 생후 70~90일된 마우스 실험에서 암컷 마우스가 수컷보다 기초 내지 산 자극 십이지장 중탄산염을 1.5배 내지 2.4배 더 많이 분비하고 있었다[37]. 이러한 분비는 ER 길항제(antagonist)인 ICI 182,780 내지 타목시펜(tamoxifen)에 의해 곧바로 사라졌고 17β-estradiol 내지 ERα 작용제(1,3,5-Tris [4-hydroxyphenyl]-4-propyl-1H-pyrazole)에 의해 재빨리 증가했으나 프로제스테론(progesterone)과는 관련성이 없었다[37]. 또한 17β-estradiol은 ERα 내지 ERβ를 발현하는 십이지장 상피세포주(SCBN cell line)에서 칼슘을 세포 내로 이동시켰고 이는 cystic fibrosis transmembrane conductance regulator와 Cl-/HCO3 anion exchanger와도 연관성이 있음이 밝혀져 십이지장궤양의 성별 차이 기전의 근거로 제시된 바 있다[38]. 또한 위궤양의 성차에 대한 실험으로는 쥐의 발과 다리를 묶은 후 냉장고에 3시간 넣어두는 스트레스를 주고 2주가 지난 후 스트레스 궤양이 발생한 정도를 비교한 결과, 스트레스를 받은 수컷 쥐는 100% 스트레스 궤양이 발생한 반면 암컷 쥐는 80% 발생해서 스트레스 궤양에 수컷 쥐가 더 취약함을 보여주었다[39].
결론적으로 역류성 식도염이 남자에 많은 이유는 하부식도조임근 등의 구조적 인자와 함께 식도 점막세포의 방어에 관련되는 에스트로젠의 항염증 작용, 밀착연접 관련 단백질 발현이 낮음에 기인할 수 있고, 이에 반해 비미란성 역류 질환이 여자에 흔한 이유는 위산에 대해 느끼는 내장과민성이 높기 때문이다. 또한 소화성 궤양의 발생률이 남자에서 높고 출혈과 천공 등 합병증 발생이 여성보다 남성에서 낮은 이유 중 하나는 에스트로젠에 의한 밀착연접 관련 단백질 발현 증가로 점막 투과성이 낮아지고 십이지장 점막에서 중탄산이온 분비의 촉진되기 때문이다. 이처럼 에스트로젠은 우리 몸 소화관 점막에 분포하는 ERβ를 통해 방어기전을 높임으로써 역류성 식도염, 소화성 궤양에 있어 성별 차이를 유발하는 요인이 되고 있는 바 향후 이에 대한 연구가 더 많아져 역류성 식도염, 소화성 궤양 발생 예방과 맞춤 치료에 응용되기를 기대해본다.
